Thank you for your interest in St. Joachim Elementary
School. In order for your child’s application to be considered,
we must be provided with the following:
Current Immunization Card
Baptismal Certificate (for Catholic students)
Evaluation form (from current teacher)
Most Recent Report card (for Grades 1-8)
Most Recent Test Scores - CTBS, SAT (for Grades 3-8)
Birth Certificate (For Kindergarten)
All information is due before the testing date. No application will
be
processed until all information has been submitted to our school office.
Process:
1. Fill out application and pay $25 application fee.
2. Give Evaluation Form with Stamped Self-Addressed Envelope (included
in application packet) to your child’s current teacher. Ask
current teacher to fill out the form and mail to St. Joachim School
as soon as possible.
3. Turn in forms (birth cert.,baptismal cert., report card, test scores
etc.)
4. Take test by appointment
5. You will be notified of your child’s status the within two
weeks of testing
6. If your child is accepted, he/she must be examined by thier physician.You
will also be asked to complete additional forms for the office. All
forms will be sent in your acceptance packet.Tuition & Parent/School
contracts, Registration, Development Fund & School Supply fees
must be submitted within 10 days of receiving the acceptance letter.
The remaining forms must be submitted to the school office by August
1, 2008.
Priority:
1. Siblings of children currently enrolled in St. Joachim
2. Children of active Parishioners
3. Children of families registered in another parish
4. Non-Catholic children, agreeing to participate fully in the life
of the School
Cost
1. Non-refundable $25.00 application fee per family due at time of
application submission.
2. Non-refundable registration fee of $250.00 is due upon acceptance.
3. One-time Development Fund payment of $200.00 for new families or
$100.00 for new sibling due upon acceptance.
4. A school supply fee of $50.00 per student due upon acceptance
